Universal Code Sheets
The NRCPR provides several documents (below) to assist your institution in collecting
data associated with the National Registry of Cardiopulmonary Resuscitation. NEW!
MET Event Records are available for download.
- Codesheet & MET Event Record: comes in 2 layouts and is designed for
use in both adult and pediatric patient populations. It should be printed on carbonless
paper so that the white form can be maintained in the medical record and the yellow
copy can be used for tracking and review of the event.
- Guidelines: designed to assist in the development of procedures related to
Codesheet completion. It will also provide content for educational activities. An
abbreviated version of these guidelines has been provided for those facilities who
want to have it printed on the back of the Codesheet.
- Resuscitation Review Sheet: quality management questionnaire that was designed
to provide the personnel who respond to each event the opportunity to document process
problems and answer specific questions regarding the patient. It is recommended
that this form be either oversized or printed on colored paper to prevent it from
being placed or maintained in the medical record.
Both the Codesheets and Resuscitation Review Sheet have many of the
data points required by the NRCPR. The format and questions were designed to facilitate
completion of documentation without requiring pre-existing knowledge of operational
definitions.
Medical Emergency/Rapid Response Teams (MET/RRT) Downloads & Links
Since February of 2006, the NRCPR has been supporting the collection and analysis
of MET (Medical Emergency Team) data. The goal of the MET (also referred to as Rapid
Response Systems or Rapid Response Teams) is to identify and provide early intervention
for medical conditions that are often present prior to a cardiac arrest, with the
goal of decreased morbidity and mortality for hospitalized patients.
